FIRST VISIT

Upon your arrival, the staff of Florida Root Canal Specialists will review your personal and insurance information. Dental insurance can be complicated! Usually, we are able to give very accurate estimates regarding co-pays and answer any questions regarding your financial information. However, to assist our office, please bring any insurance cards or other information you have. Nowadays, most insurance companies list your information by date of birth and social security number. If the insurance is in a spouse’s name, please come ready with their date of birth and social security number.

Please bring with you to the appointment:

• The referral slip given to you by your dentist
• Any x-rays or additional notes provided by your dentist
• Lists of any medications you are taking
• Any additional pertinent medical information our doctors would need to know.
